This Privacy Policy explains how ProSupply RX LLC ("ProSupply RX," "we," "us" or "our") collects, uses, shares and protects information when you visit prosupplyrx.com, create an account, complete a medical intake or purchase a product or service (together, the "Services").

Information we collect

• Account and contact details such as your name, email address, phone number, shipping address and date of birth.
• Health information you provide in your medical intake, messages to the care team, lab results and identity verification documents. Health information is handled under our HIPAA Notice of Privacy Practices.
• Payment information, which is processed by our payment provider. We do not store full card numbers.
• Usage and device data such as pages viewed, browser type, IP address and cookie identifiers, collected automatically when you use the site.

How we use information

• To create and manage your account and member portal.
• To connect you with independent, US-licensed clinicians and US-licensed pharmacies and to fulfil orders.
• To communicate with you about orders, shipping, lab results and clinical follow-up.
• To operate, secure and improve the Services.
• To send marketing communications where you have opted in. You can opt out at any time.
• To comply with legal obligations.

How we share information

We share information only as needed to provide the Services: with treating clinicians, partner pharmacies, laboratories, payment processors, shipping carriers and service providers who act on our behalf under confidentiality obligations. We never sell health information and we never use it for advertising. We may disclose information when required by law or to protect the rights and safety of our users.

Cookies and analytics

We use essential cookies to run the site and checkout. With your permission we also use analytics and marketing cookies to understand how the site is used. You can change your choice at any time using the cookie preferences link in the footer. We honor Global Privacy Control signals.

Text messaging

If you opt in, we send transactional text messages about orders, shipping and clinician communication, and, separately, marketing messages. Message frequency varies and message and data rates may apply. Reply HELP for help or STOP to opt out. Mobile information is never sold or shared with third parties for marketing.

Your choices and rights

You can access and update your account information in the member portal, request a copy or deletion of your data, and opt out of marketing at any time by contacting [email protected]. Certain health records must be retained for the period required by law.

Security and retention

We use administrative, technical and physical safeguards designed to protect your information, including encryption in transit and at rest. We keep information for as long as needed to provide the Services and meet legal, medical-records and accounting requirements.

Children

The Services are intended for adults 18 and older. We do not knowingly collect information from children.
