  • Tretinoin, azelaic acid, niacinamide and other actives are compounded into one formula tuned to your skin's concerns, whether acne, texture, tone or aging.
  • Anyone dealing with persistent breakouts, congestion, uneven tone, dark spots or fine lines who wants clinical-strength actives without stacking five products.
  • Skin intake — Photos and concerns guide which actives your compounded formula is built around.
  • Ramp-up phase — Start every third night, then build to nightly as tolerance develops. Sunscreen daily is non-negotiable.
  • Reformulate — At 12 weeks your provider adjusts strengths or actives based on how your skin responded.
  • Weeks 2 → 4 — Skin turnover accelerates. Texture begins to smooth and pores look refined. Some peeling or sensitivity is normal as the renewal cycle resets.
  • Weeks 4 → 8 — Tone evens. Hyperpigmentation lightens, fine lines soften, and overall radiance climbs as collagen synthesis ramps up.
  • Months 3 → 6 — Visible firmness and elasticity improvements. Long-term photoaging reverses with consistent use. Skin reaches a new, more resilient baseline.

A clinical primer

Skin ages on three timelines, and only one is genetic.

Cell turnover slows, collagen breaks down faster than it is rebuilt, and pigment turns uneven. Prescription actives intervene at each stage in a way over-the-counter concentrations cannot.

The underlying mechanisms
01
Turnover

Slowed cell renewal

Epidermal turnover stretches from about 28 days to over 40 with age, dulling texture and trapping dead cells and pigment.

02
Structural

Collagen degradation

UV exposure and inflammation switch on enzymes that break collagen down faster than fibroblasts can replace it.

03
Pigment

Melanocyte dysregulation

Inflammation and sun trigger uneven melanin production, which is the source of dark spots and post-acne marks.

Fig. 02 — The felt experience

You notice it in photographs before you notice it in the mirror.

Skin changes gradually enough that you adjust to it, until a photo or a certain light makes the change obvious.

  • 01 Texture that no longer looks smooth in daylight.
  • 02 Marks from breakouts that outlast the breakout.
  • 03 Lines that stay after your face relaxes.
  • 04 A routine of products that stopped working.

BONE & RECOVERY

Vitamin D

Below target, and important for bone density and immune resilience.

Tretinoin is a prescription cream from the retinoid family. It softens the look of fine lines and wrinkles by increasing collagen, the protein that forms your skin's support structure. Collagen production falls by about 1% a year from your twenties onward, which contributes to aging skin. Tretinoin also encourages new blood vessel growth in the skin, improving its color, and it fades age spots and smooths rough patches. It is the workhorse of prescription skincare.

Niacinamide is a form of vitamin B3. It calms inflammation and supports ceramide production, the building blocks of your skin barrier. It also smooths skin and reduces dark spots, which makes it a genuine multitasker.

Hyaluronic acid occurs naturally in your body and excels at holding moisture. In skincare it attracts and binds water, keeping skin plump, hydrated and youthful-looking.

Hydroquinone is a skin-lightening agent used for dark spots and hyperpigmentation. It works by slowing the production of melanin, the pigment that gives skin its color, which helps fade dark spots, sun spots, age spots and acne marks.

Hydrocortisone is a mild topical steroid that reduces inflammation, swelling, redness and itching, added when extra calming is needed.

Tretinoin comes with a few precautions. A brief feeling of warmth or stinging can occur right after application, and redness, dryness, itching, flaking or mild burning may appear during the first 2 to 4 weeks. These effects usually ease with continued use.

Topical niacinamide is generally well tolerated but may cause mild burning, itching or redness.

Hyaluronic acid is generally well tolerated because your body already produces it. In rare cases a topical allergic reaction can occur.

Common side effects of hydroquinone include redness, dryness and irritation. Hydroquinone also carries a risk of exogenous ochronosis, a gradual blue-black darkening of the skin. The risk is higher in darker skin types, with strengths above 4%, and when treatment runs longer than 3 months without a break, although it can develop at any point during treatment. Any sign of blue-black darkening means hydroquinone should be stopped immediately. Animal studies have shown some evidence of carcinogenicity, and the carcinogenic potential in humans is unknown.

Topical hydrocortisone may cause burning, itching, irritation, redness or dryness.
